If you are looking for support through Carinya Society, or looking to find out if our service is suitable for you or someone you care about please call our Acting Client Services Manager Kacie Graham on (03) 9354 3337 for a confidential discussion. Our Participant Handbook, click here to download it, will provide you with general information regarding Carinya Society and such things as our services and your rights and responsibilities. Initially we would like to understand your current circumstances and what it is that you are seeking. We find by downloading and completing the below forms our discussion can be quite focused on your particular needs.
For our current participants, families and carers, below are some additional forms that you may require.
- Click here to download the ‘Epilepsy Management Plan’ – required to support anyone who is Epileptic
- Click here to download the ‘Asthma Action Plan’ – required to support anyone who is Asthmatic
- Click here to download the ‘Treatment Sheet’ – required for staff to be able to administer medication. If staff do not have a completed form for medication it will not be administered.
Please note, we also require all medication to be in a Webster pack. Your local Pharmacy should be able to support you with this.
